Before downloading any FertiQoL PDF’s please read the following terms and conditions.

2. FertiQoL is free to use but you must acknowledge the sponsors in any publication. Please site either of the following two publications if you intend to use FertiQoL:

If you are doing research then at the end of your project we would be grateful if you could send us the sample size for your project, and means and standard deviations for each FertiQoL subscale for our monitoring purposes via email at fertiqol@cardiff.ac.uk.
Relevant population
FertiQoL is suitable for anyone who feels they are experiencing fertility problems. People who do not currently have a partner can leave blank items about the marital relationship/partnership. The optional FertiQoL Treatment module can be self-administered to people who have used fertility medical services (which includes any medical consultation or intervention).
Time frame
Responses to items are given according to 'current thoughts and feelings'.
It takes about 10-15 minutes to complete FertiQoL.

FertiQoL instructions are:
"For each question, kindly check (tick the box) for the response that most closely reflects how you think and feel. Relate your answers to your current thoughts and feelings. Some questions may relate to your private life, but they are necessary to adequately measure all aspects of your life. Please complete the items marked with an asterisk (*) only if you have a partner."
